Where does this form go? Where does the email go? It is not taking the shortcode from the theme settings for the form. I think that would be better to have it go to the gravity forms shortcode. Thanks.

I've been asking where this form goes and what it's supposed to do. As soon as I get some clear direction, I'm happy to do whatever they want with this.

I added the GravityForms shortcode and styled it to match the proof. I think we're good on this unless they want to do something like Constant Contact.
