Right now, the blue text is showing the form title and not what the Form Blue Text says, so either get rid of the blue text field and just show the form title or have the blue text overwrite the form title or show the blue text instead of the form title. I like the first option best, using the form title by default, but overwrite it with the blue title if they have something there.
