Talking with Kyle Sullivan, this may not be able to be an online form and opens Diocesan up to lawsuits and liability. We talked about this for a couple of forms. So we may want to have a talk with them about this, as it could open them up to lawsuits as well. Passing credit cards, bank account information, other confidential information across the site is not a good idea given the time allotted.

I know that the two that they have sent so far, the Auto-withdrawl and the Fair Credit Reporting Act Disclosure/Selection Background and Release Form are both a no-go for online. It depends what is on the other forms. So I will see them and run through them and if need be by KS.
