FYI - Customers are not permitted to change their bulletin shipping address on the fly.  They must contact customer support to change/adjust, or include in Special Instructions.

We are going to repeat the file type and file portions for additional files.  No mockup necessary for that, unless you want to change the design.

See the current version for example of what I mean.

Had a meeting with Gary this morning, and Curt mentioned a little bit about this - Turns out were are going back to one file at a time.  So, the repeating file upload portion is getting axed.

I will need a mockup of the "results" portion to see what you have in mind for that.

File upload page has been revised.  Working, and ready to test for design/style.

We used a button instead of radio No/Yes for additional upload.

A case for defaulting to this Sunday for "Date to Print"
-I figure 95% of the time they are uploading for the upcoming Sunday
-Having this default to something, eliminates complications below where the "Date to Print" is used
-Please consider using "this Sunday" as the default Date to print.

Remaining to-do pieces and known issues:
-Church Shipping Address in the works (Kyle)
-File name uploaded on Success (Kyle)
-Form not validated at the moment, ie File is required but allows no file (Ryan)

Side note - As admins you'll see a church dropdown.  Regular users will not see this.  This is for our support/production staff like a Shawn or Harry to be able to submit bulletins for a church.
