https://scranton.diocesanweb.org/css/home/css-volunteer-application-form/ has incorrect CSS footer

We noticed that https://scranton.diocesanweb.org/css/home/css-volunteer-application-form/ has a different footer than the rest of the CSS pages. Can you please correct this?

Also, for the CSS pages, the default template does not have the "Need Other Help?" bar. The Offices/Program template, however, does.
